Northwest Colorado Legal Services Letters, Applications and Surveys (below)
Please use these application packets ONLY if your case is located in one of the counties listed in #3 below. Before filling out and returning the application, call 1.800.521.6968 to make sure that your case is one we generally handle. We will review your application within a reasonable period of time, and will let you know if you qualify for our program and if we will be able to help you with your case.

You must complete the four steps below:
1. Print and fill out an application.
2. Print out, read and sign the Client Rights and Duties form.
3. Print out the letter from the Northwest Project Office serving your county; choose from the list below:
- Frisco (for Clear Creek, Gilpin, Gunnison, Pitkin and Summit)

- Hayden (Grand, Jackson, Moffat, Rio Blanco, Routt)

4. Click on the type of legal problem you are having below. Print and complete the questionnaire and return to the appropriate office listed in 3. above.
